Honors/AP Course Appeal Form

Click HERE for the link to the online form for Honors and Advanced Placement Course appeals.  Forms are due June 8th!  Honors and Advanced Placement courses have entrance criteria. Please review the Program of Studies before attempting an appeal. In the event that a student does not meet the entrance criteria and he or she has extenuating circumstances that should be considered in the determination of placement, this form is the vehicle for an appeal. The appeal will be reviewed by the Curriculum Supervisor and Instruction and students will be notified in writing of the decision. 


Late Qualification Form

This form will be posted in mid-June and will be due July 15th.  This form is for students who did not qualify for Honors or AP courses during the time scheduling requests were made but do qualify with their final average grade in the course.  Only students who now qualify for the Honors or AP course may utilize this form.


Permission to Disclose Disability Form

Click HERE for the form to give your Counselor permission to disclose/discuss your disability in your recommendation letter. Please note that we may not disclose this information without a signed form. 



Primary List of Colleges

Click HERE for the Primary List of Colleges form to complete prior to Junior Conferences.  Please complete this form and bring it with you to your conference.


College Prep Packet

Click HERE for a PDF file.  You must bring the entire completed packet to your counselor by June 1st in order to schedule a senior meeting for your counselor recommendation and to begin requesting transcripts.  This packet must include the following:  1. Self-Description Survey, 2. Parent Brag Sheet, 3. Student Activity Résumé (click HERE for sample résumés), 4. Final College List and 5.
